What is Long COVID?

Long COVID (also called post-COVID conditions or PASC: post-acute sequelae of SARS-CoV-2 infection) refers to a range of symptoms that persist or recur at least 3 months after the initial COVID-19 infection. 

Typical symptoms include:

  • Brain fog, difficulty concentrating, memory problems
  • Persistent fatigue or worsening of symptoms after exertion
  • Sleep disruption, headaches, dizziness
  • Anxiety, mood changes or depression
  • Sensory overload (e.g., lights/noise), tingling, or nerve-pain sensations
  • Shortness of breath, palpitations or autonomic symptoms

Even when laboratory tests, imaging, or standard neurologic exams appear “normal,” many individuals feel significantly impaired. That’s because Long COVID often involves subtle disruptions to brain networks and the nervous system that evade conventional testing.

How Long COVID Affects the Brain

Recent research suggests that Long COVID isn’t just “tiredness after an infection”—it frequently involves the brain and nervous system, including:

  • Neuroinflammation: persistent activation of microglia and inflammatory signaling in the brain. 
  • Altered brain structure, perfusion, and connectivity: Imaging studies (MRI, SPECT, PET) show changes in blood flow, connectivity, and metabolic activity in people with cognitive/“brain fog” symptoms post-COVID. 
  • Dys­autonomia and network dysregulation: Many patients report orthostatic intolerance (e.g., POTS), rapid heart rate, and dizziness, which suggests involvement of central autonomic networks. 

 

Together, these suggest that brain networks may remain in a “dysregulated” state after COVID: some areas underactive, others overcompensating, and overall network synchronization reduced. This is precisely where neuromodulation (such as rTMS) may play a role.

Current Approaches and Why They Often Fall Short

Because Long COVID presents so variably, current management is largely symptom-based:

  • Treating sleep problems, mood or anxiety, pain, or headaches
  • Physical/occupational/speech therapy for cognitive and functional deficits
  • Pacing and energy management for fatigue and post-exertional worsening

 

However, for many people, these approaches help only partially, and the core cognitive or brain network dysfunction persists. That’s why researchers are exploring neuromodulation—treating the brain network itself rather than only downstream symptoms.

What is MeRT?

At Brain Treatment Center Michigan, we offer MeRT, which is best understood as a personalized, EEG-guided form of rTMS.

Here’s how it works:

  • We collect a qEEG (quantitative EEG) to map your brain’s electrical activity and identify areas of slowing, overactivity, or abnormal network synchrony.
  • We also record an EKG to time the stimulation with your own brain/heart rhythm for optimal effect.
  • Based on your unique brain map and symptoms, a customized rTMS protocol is designed: target location(s), frequency, intensity, and number of pulses/sessions.
  • The rTMS device then uses magnetic pulses to gently stimulate the target brain area(s), aiming to reset or modulate network activity, promote neuroplasticity, and improve synchronization.

While standard rTMS is FDA-cleared for major depressive disorder and obsessive-compulsive disorder, its use in Long COVID is off-label—but increasingly supported by research and clinical experience.

Important note: MeRT is not designed to treat acute respiratory, cardiac, clotting or organ-damage issues from COVID. Those issues require appropriate medical specialist care.

Initial Consultation

If you’re interested in MeRT, the first step is to contact our New Patient Coordinator. She’ll walk you through what to expect, review treatment protocols, discuss costs and payment options, and answer any questions you may have about whether MeRT is a good fit.

Brain Scan & Doctor Consultation

If you decide to move forward, we’ll schedule your initial two visits.

Visit 1: A non-invasive qEEG brain map and EKG to assess your brain’s electrical activity and rhythm.

Visit 2: A consultation with the physician to review your results, discuss what the findings mean, and determine whether MeRT may benefit your Long COVID symptoms.

Personalized Treatment Plan

If you’re a candidate for MeRT, the physician will create a tailored treatment protocol based on your brain map and symptom profile. This includes selecting the target brain regions, stimulation frequency, intensity, and recommended number of sessions—typically several sessions each week over a 4-6 week period.

Treatment Sessions

During each session, you’ll sit comfortably while a magnetic coil delivers gentle pulses to specific areas of your scalp. You may feel light tapping or hear clicking sounds. Sessions last about 30–40 minutes, and you remain fully awake, with most people returning to their normal activities afterward.

Ongoing Monitoring & Remapping

As treatment progresses, we’ll periodically reassess your symptoms and repeat qEEG mapping to monitor changes in brain activity. This allows us to adjust your protocol as needed to ensure the most effective results.

Safety & Side Effects

  • rTMS has been used clinically for decades with a strong safety record when delivered by trained professionals.
  • Common side effects: mild scalp discomfort, temporary headache, fatigue after sessions. Serious adverse events are rare when proper screening is performed. 
  • The early Long-COVID rTMS studies reported no serious adverse events; participants tolerated the treatment well. 
  • As with any treatment, we’ll review your full medical history and ensure there are no contraindications (for example, certain implanted metal in the skull, active seizure disorder, some cardiac devices, etc.).

Who May Be a Candidate?

You may be a good candidate for MeRT if you:

  • Have a history of confirmed or suspected COVID-19 infection and have persistent symptoms 3+ months later (i.e., Long COVID)
  • Are experiencing significant brain-based complaints (fog, memory issues, mood/sleep problems, cognitive fatigue) that interfere with your daily life
  • Have undergone or are undergoing appropriate medical evaluation (for example, pulmonology, cardiology, sleep medicine) and are looking for additional options to improve brain health
  • Can commit to multiple treatment sessions (typically 4-6 weeks or more)

You may not be a candidate if you:

  • Have certain contraindications (e.g., uncontrolled epilepsy, metal implants in or near the brain area, some cardiac devices)
  • Are still in the acute infectious or highly unstable phase of illness
  • Have primary symptoms that are not brain-based (for example, solely lung scarring, clot disorder, or advanced organ damage). In those cases, the underlying specialist care is essential.

Is MeRT FDA-approved for Long COVID?

No. The equipment used for MeRT is FDA-cleared for major depressive disorder and OCD, but its use for Long COVID is off-label and experimental.

Absolutely. MeRT is intended to be an adjunct to your comprehensive care. We encourage you to coordinate with all your providers.

Treatment plans vary based on your brain map and symptoms. Many patients undergo multiple sessions per week over 4-6 weeks or more, followed by optional periodic maintenance. We’ll discuss your individualized plan when you come in.

What Results Can Patients Expect?

While every person is unique, here is what you might reasonably hope for:

  • Early improvements often show in sleep, mood or clarity of thinking within the first few weeks.
  • Cognitive improvements (memory, concentration, multitasking) may take longer and often occur gradually.
  • Because Long COVID is a complex condition, MeRT is best seen as a component of a broader recovery plan (sleep hygiene, pacing, therapy, lifestyle support) rather than a “magic bullet.”
  • In published Long COVID rTMS studies, many participants reported improvements in cognition and mood; fatigue results are more variable. 

 

We emphasize: We cannot guarantee a specific outcome, but our goal is to help your brain work better, feel clearer, and regain more of your life.
